Requesting a Birth Certificate
There are five ways for you to get your Illinois birth certificate if you were born inside Cook County:
1. Order Online (Birth Certificate, Marriage record, Death Certificate) by clicking here.
2. Order by phone by calling toll free 1-866-252-8974
3. Write to the County Clerk
Mail your request to:

Include the following items:
- A completed Birth Certificate Request Form
- A photocopy of your photo identification (Driver's License, State ID, etc.)
- A check or money order payable to the Cook County Clerk for $15 (additional copies are $4 each)
- A self-addressed stamped envelope
4. Go to a local Currency Exchange

Note that there is a $5 charge for records ordered at currency exchanges
