Requesting a Birth Certificate

There are five ways for you to get your Illinois birth certificate if you were born inside Cook County:

1. Order Online (Birth Certificate, Marriage record, Death Certificate) by clicking here.


2. Order by phone by calling toll free 1-866-252-8974


3. Write to the County Clerk

     Mail your request to:

    Cook County Clerk David Orr

    Bureau of Vital Records

    P.O. Box 642570

    Chicago, IL 60664-2570


    Include the following items:

  • A completed Birth Certificate Request Form
  • A photocopy of your photo identification (Driver's License, State ID, etc.)
  • A check or money order payable to the Cook County Clerk for $15 (additional copies are $4 each)
  • A self-addressed stamped envelope

4. Go to a local Currency Exchange

    Call 1-847-759-8905 for Currency Exchange hours and locations in Chicago and  

    suburban Cook County or click here.

    Note that there is a $5 charge for records ordered at currency exchanges
